Can you believe we are already in the second half of the semester? With that said, I would like to get your feedback on how the course is going with regards to the material/content and teaching. 

Please take a few minutes to complete this brief mid-semester course evaluation. This evaluation is anonymous; that is, your name or any other identifying information will not be associated with the survey responses. Please feel free to leave your candid and honest responses. Finally, while I greatly appreciate and value your feedback, I may not be able to make all, if any, of the suggested changes suggested within the next few classes.
