Reaction to Bromfield's "Teens in Therapy"

I found Bromfield's delineation of short-term goals as usually more concrete and behavioral and long-term goals as pertaining to personality, character, and identity (p. 26-7) really helpful in thinking of how to conceptualize a case and then start to work on it. His list of therapeutic mechanisms (listen, confirm what they are going through, hold space, offer insight, etc.) start to give me an idea of HOW to work on the client's goals, but I would love more information about what that would look like in a session for a teenaged client where the primary mode of therapy is talking. Is asking them about how they are doing, or what they'd like to talk about today, too vague and possibly leading us down paths that don't relate to the goals? Or will starting a session like that naturally start to lead us to working on the goals? In other words, how do we start working on the goals?
